To find out more about our SRS at Acacia Place Supported Residential Services, please call or email us. We welcome you to come and visit our home. If you’re interested in moving to our facility in Victoria, Australia, there are a few steps to follow:

1.Contact the Manager on 7981-5922 Monday to Friday 9-5 for a discussion

or

Fill out an SRS referral form from your health support contact and send via email or fax.

3.The Manager will advise if we are able to provide the type of support you need and contact you or your support person.

4.If we find that we are able to offer you a home with us, a Residential Service Agreement and support plan will be developed with you.

5.Family members, case managers, and other support people are welcome to contact us and be part of the discussions.
